Health Forms

A current (within the last 13 months) copy of your student’s physical needs to be submitted annually. For example, if your student's last physical was in September 2024, that exam is valid until October 2025. As we are sensitive to the fact that some insurance companies will only pay for one exam per calendar year, we give families a thirty day grace period from the date of the last exam.  All health forms must be completed and uploaded online to Bryn Mawr's Magnus Health system, accessible via the Parent Portal, by July 1st.  Parents will receive access to the Magnus Health system in mid-April.
